Medical Care at School, at No Cost from  San Bernardino City USD

HAZEL HEALTH ANNOUNCEMENT

Dear Sierra High School Families,
We want to make sure you know about the health care services available at school for your student. This service is provided at no cost to you. Our district is partnering with Hazel Health to provide this additional layer of health services so when your child is sick at school, they can be seen by a doctor/provider. 

In order for your student to use this health care service, the consent form needs to be filled out.

Please complete the consent form online at my.hazel.co/sbcusd. Or you can pick up a paper consent form from the front office.

 Why is your consent so important?

If your student’s not feeling well, they can see a doctor/provider right away who can evaluate their symptoms and provide the appropriate type of care so you don’t have to leave work during the day. Care may include:

  • If you student has a fever, or is experiencing pain or discomfort, the doctor/provider can give over-the-counter medicine.
  • Hazel helps with prescriptions your student may need, so if your student has pink eye for example, Hazel can call in a prescription for you.
  • Hazel can conduct health screenings to help your student connect with any needed school or community based resources.

No insurance, no co-pay, no cost


San Bernardino City Unified School District and Hazel have a shared goal: To ensure every student has access to quality healthcare, regardless of their financial, insurance or immigration status. Why? Because healthy students learn better. 

Hazel connects students to healthcare providers with the touch of a button on an IPad. Students get quality care, without leaving school. Parents don't miss work, and no need for a bus ride to the doctor's office (or the ER).

Services include: 
  • Urgent care
  • Health Screenings
  • Patient Education 
  • Provider ordered over- the- counter medication and/or prescriptions
  • Referrals to health care professionals, school and community resources


How Hazel Works

Consent
With parent/guardian consent, students can participate in Hazel's health program.
Connect
when a student is sick, they'll visit the on-site Hazel clinic. Using cutting- edge video technology, they'll talk to a licensed healthcare provider. 
Care
Care is provided and may include over-the-counter medications given at school and/or prescriptions called in to a local pharmacy.
Contact
Parents/guardians are contacted to explain the visit, medications, diagnosis and recommendations for further care.

Q: How much does Hazel cost?
A: Hazel is provided by your School District at no cost to families. 

Q: How many visits do students get?
A: There is no limit on the number of visits. 

Q: How does this affect insurance?
A: A visit with Hazel will have no impact on your coverage. You will not receive a bill for any services provided. 

Q: What if my child already sees a doctor?
A: No problem. Hazel is a complimentary service at school for students and will not affect any other medical care. In fact, Hazel is happy to work with your current doctor and provide any information from a Hazel visit. 

Q: How do families sign up for Hazel?
